AT A GLANCE
Details, the change requested, effective and freeze dates, an optional reason and a signed declaration.
Agreement checked, amounts owing, final debit date, access removed, confirmation sent and refund logged.
Record reasons consistently so you can see why members leave and what might have kept them.
BEFORE YOU USE IT
Notice periods, fees, freeze rules and cooling-off rights come from each member's agreement, the Australian Consumer Law and, in some states, fitness industry rules. The form deliberately states none of them. Check the agreement before confirming dates or amounts, and make cancelling straightforward.
If the change leads to a refund, record the request, evidence and approval.

That depends on your membership agreement and the law that applies. Many clubs accept requests in person, by email or through an app. The form records how the request arrived.
Yes, but keep it optional and never make it a condition of cancelling. Reasons recorded consistently show you what to fix.
A freeze pauses the membership for a set period under your agreement's freeze rules; a cancellation ends it. Record freeze start and end dates so access and billing restart correctly.
