AT A GLANCE
Fix the tag where members see it before they use the equipment, and isolate the item where you can.
Record the item in the register with its fault, the date and the maintenance job reference.
Remove the tag only once the repair has been checked, and record who returned it to service.
WHY A REGISTER
Out-of-order signs tend to stay up for weeks when nobody owns the fix. The register gives every tagged item an owner, a job and a date, so you can see what is out of action at a glance.
In ClubPilot every fault gets a numbered job, an owner, quotes, approvals and verified completion.

Only the person or role named on the tag, under your procedure, after the repair or check is complete. Write that name or role on every tag.
Where you safely can, yes. Isolate it according to your procedure and the manufacturer's instructions. A tag tells people not to use it; isolation stops them.
Print on A4 and cut along the dashed line for two tags. Laminating them or using a plastic sleeve helps them last on cardio consoles.
